Urban Air Adventure park fails health inspection

The health inspection world grades a score below 70 as failing, and one establishment received a failing score from inspections conducted on Aug. 18–22.

Worst scores in this week’s health inspection

This week’s lowest health inspection score was a 60 earned by an Urban Air Adventure Park, located at 1207 Robert C. Daniel Parkway, in Augusta.

During the inspection, the inspector noticed no certification for food safety management, which resulted in a four-point deduction.

“Observed no Certified Food Protection Manager for the establishment. Obtain certification w/in 60 days of this inspection,” the report stated.

The inspector also reported observing employees not following proper conduct at work, resulting in another nine points taken from the health score. 

“Observed an employee not wash hands and change gloves between taking food orders and prepping food. Observed employee adjust clothing without washing hands afterwards,” the report stated.

Another four points were deducted from the health score for the inspector finding no paper towels at the employee handwashing sink, with the next four sessions being reduced by 

“Observed hand washing sinks not supplied with paper towels throughout the kitchen. Ensure hand washing sinks are supplied w/ paper towels at all times” the report stated. 

After losing another three points for observing multiple food items stored on the floor throughout the kitchen, the inspector deducted three more points for finding out of date food.

“Observed several food items (cheddar & sour cream chips-8/12/25, garlic sauce-2/9/25, and nacho chips-7/29/25) stored past date markings,” the report stated. “Monitor date markings and ensure food items are properly discarded.”

The establishment lost three more points for having high activity bug activity in the kitchen.

“Observed fruit flies throughout the kitchen,” the report stated. “Consult with a licensed pest control operator to get rid of fruit flies.”

Throughout the inspection, in addition to losing points for bug activity, the inspector docked another three points for witnessing employees not wearing proper gear.

“Observed an employee not wearing a hair restraint during active food prep,” the report stated. “Employees must wear hair restraints during active food prep.”

The second lowest score of the week was a 71 given to a Tony’s Express Restaurant, located at 4471 Columbia Road, Suite A–1, in Martinez.

The inspector deducted four points for noticing improperly stored food items.

“Observed multiple food items stored uncovered throughout establishment (Noodles, Broccoli, Zucchini, Carrots, Mushroom, Seafood, etc. in walk-in-cooler, walk-in-freezer, and prep coolers)” the report stated.

During the inspection, the inspector deducted another four points for finding employee drinks out of the designated beverage area of the kitchen.

“Observed employee drink and call phone stored on prep table where food was being prepped,” the report stated.

The inspector found more food items improperly stored, which resulted in another nine point reduction.

“Observed food items in prep coolers and walk-in-cooler that were not already on ice above 41 degrees,” the report stated.

For finding even more food left out and unlabeled, the inspector deducted another seven points.

“Observed no date marking of opened, stored, or prepared food items,” the report stated. “Observed packs of alligator tail thawing/kept in pan with fish and water when pack says to keep frozen and thaw slowly under refrigeration condition.”

For not seeing any thermometers in any of the cold holding units and noticing unclean gaskets, the score was reduced by another four points.

After observing food stored on the floor, the inspector took three more points off the establishment’s score.

“Observed food stored on floor in WIF and sauces on floor in dry storages. Don’t reuse open metal cans,” the report stated. “Store at least 6 inches above ground and place product from opened metal cans into storage containers.”

Following the Tony’s Express Restaurant, the third lowest score of the week was a 77 given to a Forest Hills Grill, located at 1500 Comfort Road, in Augusta.

The inspector deducted four points for noticing no sanitizer entering the dish machine.

“Observed no sanitizer entering dish machine and none available at three compartment sink,” the report stated. “Repair to be able to properly sanitize dishes.”

During the inspection, the inspector deducted another four points for noticing build up on kitchen equipment.

“Observed black build up in Sprite nozzle of soda fountain,” the report stated.

Another point was docked from the establishment’s health inspection score due to the inspector witnessing the hand sink pulling away from the wall.

“Repair to keep sink from detaching from wall,” the report stated.
